Port notes and original source

Ported from CAI~1.JAV and calculate1.java (October 31, 2003), by Eric Tedjamulia and Edward Tan. Ten questions use numbers 0-9; a correct answer on any of three attempts earns ten points. Browser controls replace the original Swing dialogs.

Original Java archive. The original source and artwork were preserved. This version runs in JavaScript and HTML canvas without a Java plugin. A local copy can also be opened directly from its index.html file.
